We serve everyone, regardless of income or insurance. Our financial services team can help you find the plan that makes it easiest to afford care. We accept most major insurance plans and can also enroll qualified patients in our affordable Discount Program.
Count on Your Care Team
Mountain Park’s care teams tailor healthcare to your specific needs. A care team is made up of your primary care doctor, medical assistant, pharmacist and other integrated care team members.
As your care team gets to know you, you’ll spend less time repeating your health history and more time working with your provider on a personalized care plan. Mountain Park is a Joint Commission-accredited, patient-centered medical home.
You won’t leave our clinic until you get the care you need. Our registered dietitians, behavioral health consultants and clinical pharmacists will visit with you in the same exam room as your doctor. No need for new appointments and figuring out how to get back to the doctor’s office – we’ll do that for you.

A Message to Our Patients
While checking in to your next appointment at Mountain Park, if you are 18 years or older, you will be asked a series of new questions that will help us better care for you.
These questions focus on issues that can be sensitive, including: sexual orientation (emotional or romantic attraction to others), gender identity (if you identify as a male, female, blend of both or neither), age, race, ethnicity, and information about your living situation.
All of these responses are completely confidential and typically will only need to be answered once a year. They are required for us to ask as a community health center, but you always have the option to decline or not answer the questions you don’t feel comfortable answering.
By completing these questions, you help us understand you as a patient and how we can provide better care for you.
