At Mountain Park, we tailor healthcare to your specific needs. You won’t leave our clinic until you get the care you need. Our registered dietitians, behavioral health consultants and clinical pharmacists will visit with you in the same exam room as your doctor. No need for new appointments and time spent traveling to various locations.

What to Bring to Your Eligibility Appointment

During your appointment with Mountain Park’s Financial Services team, we will help find the best programs for you. Our team members will ask several questions to discover what you and your family are eligible for. Please be assured that all responses will be kept completely confidential.

Discounted/sliding fees are based upon federal poverty guidelines, and patient eligibility is determined by annual income and family size. Schedules are established and implemented to ensure that a non-discriminatory, uniform, and reasonable charge is consistently and evenly applied.

Patients are required by law to provide documentation in order to determine eligibility for AHCCCS and Mountain Park’s Sliding Fee program. Click here to see what documents you will need to bring with you.

Most Forms of Insurance Accepted

Mountain Park Health Center accepts most forms of insurance. Our Financial Services Team will work with you directly to ensure we find a way to make getting the care you need easy. Mountain Park’s Sliding Fee program and AHCCCS are just two of the options available. We also work directly with state and federal programs as well as the Department of Economic Security.

If you are uninsured or under-insured, we will work with you to find a program that meets your needs. With our commitment to providing high-quality, affordable, preventive healthcare we want to make sure you get the healthcare you need.

A Message to Our Patients

While checking in to your next appointment at Mountain Park, if you are 18 years or older, you will be asked a series of new questions that will help us better care for you.
These questions focus on issues that can be sensitive, including: sexual orientation (emotional or romantic attraction to others), gender identity (if you identify as male, female, blend of both or neither), age, race, ethnicity, and information about your living situation.

All of these responses are completely confidential and typically will only need to be answered once a year. They are required for us to ask as a community health center, but you always have the option to decline or not answer the questions you don’t feel comfortable answering.
