Mountain Park Health Center works with the communities it serves to sustain and improve health by providing affordable primary care.

Mountain Park Health Center is a nonprofit community health center. For more than 40 years we have been providing whole-patient care for the entire family. Each year we keep nearly 100,000 of our neighbors in Goodyear, Tempe and Phoenix healthy.

Keeping You Healthy

Mountain Park believes in being proactive about your health. We know that health education, screenings and access to affordable medications help prevent serious health problems. Proactive healthcare can also help manage chronic conditions, like obesity and diabetes, and detect diseases in the most treatable stages. We are here for you anytime you need.

Keeping Healthcare Affordable

Our caring staff is here to help you navigate AHCCCS or the health insurance marketplace. We can also help you apply for SNAP (formerly called food stamps) and TANF (cash assistance). If you don’t have health insurance, we will work with you to make healthcare affordable through our Sliding Fee Discount Program and other resources.

Creating a Patient Centered Medical Home

We always put our patients first. As a Patient Centered Medical Home, Mountain Park offers all the care you need in one single place. You’ll have access to an entire Care Team of primary care physicians, physician’s assistants, nurse practitioners, clinical pharmacists, behavioral health consultants and registered dietitians all in one exam room.
