Our Baseline Clinic Pharmacy is an on-site pharmacy available to our patients. Until May 11, 2025 we are located on the second floor of the Baseline Clinic 635 E. Baseline Road in Phoenix, Arizona. Starting May 12, the Baseline Pharmacy will be located across the parking lot at our newly renovated Jesse Owens Clinic.

We are open Monday – Friday, 8:00 a.m. – 8:00 p.m. and Saturday 8:30 a.m. to 4:30 p.m.
We understand managing your prescriptions can be difficult. We work closely with your Care Team to help manage side effects or drug interactions. We are also available to answer any questions you, or your family may have. Our primary goal is to provide you with quality care. We are a part of your fully integrated health care team.
