Our Baseline Clinic Pharmacy is an inpatient pharmacy available to our patients. We are located on the second floor of the Baseline Clinic 635 E. Baseline Road in Phoenix, Arizona. We are open Monday – Friday, 8:00 a.m. – 8:00 p.m. and Saturday 8:30 a.m. to 4:30 p.m.

We understand managing your prescriptions can be difficult. We work closely with your Care Team to help manage side effects or drug interactions. We are also available to answer any questions you, or your family may have. Our primary goal is to provide you with quality care. We are a part of your fully integrated health care team.
