Am I eligible for AHCCCS (Arizona’s Medicaid)?

AHCCCS insurance helps cover the cost of primary care, behavioral health, prenatal services, prescriptions and more; often with little or no out-of-pocket costs.

You may qualify if you:

  • Are an Arizona resident
  • Meet income limits based on household size
  • Are a U.S. citizen or legal resident

Commonly eligible groups include:

  • Children
  • Pregnant people
  • Low-income adults
  • People with a disability
  • Seniors (65+)

What is the Sliding Fee Discount Program?

Our Sliding Fee Discount Program provides reduced-cost services based on household income and size.

You may qualify if:

  • You are uninsured
  • You don’t qualify for AHCCCS
  • You have high deductibles or limited coverage

How do I start the financial screening process?

What do I need:

  • Proof of income (ex: recent pay stubs, tax returns, benefits letter)
  • Household size
  • ID and verification of address (ex: utility bill, bank statement, rent or morgage documents)
  • Other items as requested

Worry Less About Your Medical Bills

Schedule your appointment today to apply for AHCCCS or our Sliding Fee Discount Program. Insurance coverage gives you access to routine physicaIs and medical appointments when you’re not feeling well without the worry of huge medical bills. If you do not qualify for either program, we offer self-pay rates starting at $150/visit due at the time of your appointment. Self-pay patients will be billed for all services during the visit that exceed the $150.
