What is proxy access?

Proxy access allows you to view electronic health records for someone other than yourself. Proxy access comes in two forms: complete access and restricted access.

How does a patient assign a proxy?

The patient can set a proxy by asking any front desk employee at any of our clinics.

Parents/legal guardians who sign up for our patient portal will be given complete access to their child’s health information record from birth until their child’s 13th birthday. You can sign up for our patient portal by visiting the front desk at any of our clinics.

What is the difference between complete access and restricted access?

Complete proxy access allows you to see health information like upcoming appointments, medications, lab results, immunization records and messages from providers. See full chart below.

Restricted proxy access allows you to see certain parts of a patient’s health information record while complete proxy access allows you to see the entire record.

Why can’t I automatically have access to my teen’s health record?

To comply with privacy laws, you will be given complete proxy access to your child’s health records until they turn 13. Complete or restricted proxy access for any patient over 13 years old must be authorized by the patient. If your child would like to authorize complete or restricted access, they can do so by asking a front desk member at any of our clinics.

Once your child turns 18, proxy access to their account will be removed. In order to reinstate access, your child will need to reauthorize your proxy access with a front desk employee at any of our clinics.

Can I have proxy access for an adult?

Yes, as long as the patient authorizes your access (complete or restricted) in person with one of our front desk employees.

Can I have proxy access to more than one member of my family?

Yes! Depending on the age of each family member, a separate authorization may be required. You can access all your proxy accounts on a single device.
