At Mountain Park Health Center, quality is more than a goal. It is a commitment we make to every patient, every day. We are proud to share that we have earned six Community Health Quality Recognition (CHQR) badges, a national recognition that reflects our dedication to delivering high-quality, patient-centered care. These six CHQR badges are the most earned by any Federally Qualified Community Health Center (FQHC) in all of Arizona! Among more than 1,500 FQHCs nationwide, only 7% earned six or more badges, placing Mountain Park Health Center among the nation’s top-performing health centers.
These badges recognize health centers that demonstrate strong performance in areas such as clinical quality, patient outcomes, access to care, and the use of data to improve health. Community Health Quality Recognition badges are awarded based on performance measures and reporting standards that help ensure patients receive the best care possible.
Earning these recognitions takes the work of an entire team. From our doctors and nurses to medical assistants, pharmacists, behavioral health consultants, registered dietitians, and support staff, every team member plays a role in helping our patients achieve better health outcomes. At Mountain Park, quality improvement is a part of everything we do, from care coordination and patient safety efforts to expanding access and strengthening the overall patient experience.
Our commitment to quality is also reflected in our long-standing focus on accreditation and patient-centered care. Mountain Park has maintained national quality standards through programs such as The Joint Commission accreditation and Patient-Centered Medical Home (PCMH) recognition, demonstrating our dedication to quality, safety, care coordination, and access to care.
