Mountain Park has on-site pharmacies at our Maryvale and Baseline clinics to help make your visit to our clinic convenient and efficient. Pharmacists are medication experts and are available to answer any question you have about your medications. They work directly with your provider to ensure your medications are not delayed!

Your Care Team also includes a Clinical Pharmacist who can help you navigate through multiple medication therapies and support your health goals.

Click Continue once you have entered all your refills and you will receive a message once your prescription is ready.

Refills are available by walk-in

After you have a visit with your Care Team at our clinics you can stop by the pharmacy before you leave to grab your prescription. In the area? Come by and get your prescriptions. Our friendly staff is here to help you have the best outcomes!

Notify the pharmacy three days prior to running out of medications. The pharmacy will contact your provider for further refills.

Should I ask the pharmacist to review my medications?

YES! We want to make sure that you know the best time to take your medication—to get the best results—especially when it is a new medication. The pharmacist will review common side effects, any other important medication information and answer your questions.

How can I meet with a Clinical Pharmacist?

A

Your Care Team consists of a multispecialty team including behavioral health, nutrition, and clinical pharmacy services. The clinical pharmacist can review your medications for multiple conditions such as anticoagulation, diabetes, cholesterol, and high blood pressure. Just let your provider know that you would like to meet with one and you will be scheduled.
